the story is so confusing that it makes no sense. Writer trying to keep things mysterious backfires greatly.

Look at Hollywood movies 1970's and earlier smoking was used practically a standard prop for coolness, maturity, or/and sophistication like they are used in China now. Of course we don't see it as that anymore but something disgusting, addict, or pathetic. Different times, different views.
